Who is eligible to use this program?

UCare plan members with Minnesota Senior Health Options (MSHO), UCare Connect + Medicare, and UCare Advocate Choice and UCare Advocate Plus (HMO I-SNP) are eligible for Caregiver Assurance.

Either the UCare plan member themselves or their designated caregiver—a single individual—may use this program. When calling, the designated caregiver must have the member’s plan information available, including the member Name, UCare ID and Date of Birth.

Does my Personal Care Assistant (PCA) qualify to use the Caregiver Assurance program?

Individuals who are not considered a professional PCA paid through an agency to provide your PCA care are eligible to use Caregiver Assurance. Access to Caregiver Assurance is available to a spouse or a parent who may qualify to be paid through Community First Services and Supports (CFSS).

How can I get started?

Call 612-672-7996 to speak with a Caregiver Advisor or contact your UCare care coordinator for additional information about the program. You can connect with your UCare care coordinator by calling UCare Customer Service at the phone number located on the back of your UCare member card.

Are interpreter services available for non-English speaking members?

Yes! The Caregiver Advisors will help to coordinate an interpreter to join all conversations if necessary. Interpreter services will be provided by UCare.
