Caregiver AssuranceTM for UCare Medicare with M Health Fairview and North Memorial Health, Care Core and Care Wise Members
Some people need help living at home — getting meals, prescriptions, house cleaning, shopping, transportation, and more. Sometimes family members provide this caring support, but even family members can become exhausted, stressed or not know where to get advice. They may need expert information, counseling or referrals to community resources. Now you can ask UCare and M Health Fairview for help.
M Health Fairview’s Caregiver AssuranceTM is a well-being program for members and the people who care for them – at no additional cost. When you call our program, we will pair your caregiver (a family member, or whomever cares for you) with one of our advisors—a dedicated social worker trained in aging and caregiving—who will listen to their concerns and needs. They will offer suggestions, support, and connect your caregiver to low- or no-cost resources in your community to help you both get what you need. Your Care Core or Care Wise plan includes unlimited calls to your caregiver advisor for you or the person who helps take care of you*.

Caregiver Advisors are available by phone Monday – Friday from 9am – 5pm.
*A single family member or caregiver may take advantage of this benefit on your behalf. When calling, they will need your UCare member information to sign-up for the program.
